Effective 7/1/06 the expanded NVA website will allow members to:

- Review a plan design description listing the major areas of vision coverage
- Determine when you or your dependents are next able to obtain covered services
- Print direct reimbursement claim forms
- View benefit use information
REMEMBER: Obtaining vision care services from a non-participating provider will result in unnecessary out of pocket expense. If you select a non-participating eye care provider, you will be responsible for one hundred percent of the cost at the time of service. To obtain reimbursement you must submit a copy of the itemized receipt along with a note containing your name, social security number or a photocopy of your plastic identification card to NVA at the above address.

Important, you have the right to obtain your contact lens prescription from your eye care professional! In 2004, a Federal Law, the “Fairness to Contact Lens Consumers” Act went into effect, mandating that patients be given their prescription as soon as they’ve been fitted. The law requires sellers to verify prescriptions and gives prescribers a reasonable amount of time, 8 business hours, to respond. It also established that a contact lens prescription will expire within one year or at the time set by State law, whichever is greater.
